Below is information that explains how Sheena Rydings Psychological Therapy complies with the GPDR regarding what data I hold and how and why I store, process and dispose of it.

 

WHAT DATA DO I COLLECT

    •    Client’s personal details (name, date of birth, gender)

    •    Contact details (address, telephone number, email- plus permission to leave voicemail message)

    •    Client’s private therapeutic notes including background information, therapy history, medical conditions relevant to therapy, prescribed      medication, summary of sessions, psychological difficulties.

HOW DO I STORE YOUR PERSONAL INFORMATION

 

Storage Methods

    •    Paper: written notes. All client’s details are in a locked filing cabinet with personal contact details stored with therapeutic notes

    •    Email/SMS: Your email address and correspondence will be stored in my email account (currently ipage webmail) by nature of you contacting me. Your telephone number may be stored in my SMS should we exchange messages in this way. Electronic correspondence will also be held by the corresponding app (SMS, Webmail)

    •    Website: None of your personal information is stored on my website, other than to momentarily collect and send to my webmail account for purposes of our initial contact if you have chosen the ‘contact me’ option on the website.

Documents held

    •    Paper: Contact sheet, assessment record, GPDR agreement, therapy contract agreement, brief session notes.

    •    Electronic: Contact name and telephone number, email/ SMS

HOW DO I SHARE/PROCESS YOUR PERSONAL INFORMATION

 

I will under no circumstances share any personal details with any third party unless legally obliged to do so. I seek regular clinical supervision with another qualified therapist. The process is for my clinical practice. In order to protect your privacy, the therapist will not know you personally nor professionally. I will refer to you by your initials and I may refer to your information verbally when it’s helpful to my professional process.

Emergencies

If your health is at risk (provided I have your consent) I may share your contact information with an emergency healthcare service (e.g. mental health crisis team, your GP). If I have become aware of your intent to cause harm to another person/ organisation (e.g.terrorism), the law may require that I inform an authority without seeking your permission. In such a situation, the law may require that I share your personal information without your knowledge.

Erasing your information

When we have finished working together, I will erase electronic copies of your information and correspondence within one month.

I will hold on to your written information for up to 7 years past the end of our working together. This is so that I have a reference of our work in situations such as you returning to therapy in the future. After this time has passed, I will shred the written information.

 

YOUR RIGHTS

You have the following rights:

    •    To be informed what information I hold (i.e. this document)

    •    To see the information I hold about you

    •    To rectify any inaccurate or incomplete personal information

    •    To withdraw consent to me using your personal information

    •    To request your personal information be erased when it is no longer necessary to hold it.

 

All the above complies with the General Data Protection Regulation using contract as a lawful basis for processing the data I hold.

I do use electronic forms on the website making use of an available ‘forms module’ where you can contact me via this form, this has a number of built-in features to help ensure privacy. I also aim to use secure forms where appropriate.

Like most websites, we make use of analytics software in order to help us understand the trends in popularity of our website and of different sections. We make no use of personally identifiable information in any of the statistical reports we use from this package. We use an analytics package via ipage webhosting who provide details of their privacy policy on the ipage website.

Social media

We use social media plugins on our website that allows you to connect with social networks in various ways. For these to work the following social media sites including; LinkedIn, Facebook, Twitter; and instragram will set cookies through our website which may be used to enhance your profile on their site or contribute to the data they hold for various purposes outlined in their respective privacy policies.

If your complaint is not resolved to your satisfaction and you wish to make a formal complaint to the Information Commissioner’s Office (ICO), you can contact them on 01625 545745 or 0303 1231113.

Reviewing the Policy

This policy will be reviewed on an annual basis and as and when necessary following any legislative changes or relevant best practice recommendations.
